Dear Colleagues,

As part of the ongoing effort of the NYSILC Housing Institute to address the housing crisis for New Yorkers with disabilities, we are conducting a statewide housing needs survey and would greatly appreciate your input! While we all know that there is a shortage of accessible, affordable and integrated housing, we need more than anecdotal evidence of the problem. Your participation in this survey will assist in documenting the housing crisis.

Use the mouse to click the appropriate response, or responses, per question. The mouse also allows you to check, or uncheck, a response by clicking a box once to check it or twice to uncheck it.

When you come across a question that requires a response, simply enter the appropriate information in the box provided and proceed to the next question. After you have completed the survey (there are 36 possible questions), click the “done” link at the bottom of the survey screen.

Your survey will have been successfully transmitted if the screen redirects you to the SurveyMonkey home page. If this happens, close yourself out of the site by clicking the “x” in the far right-hand corner of the screen. Thank you for your time and interest related to this important topic. NYSILC will analyze the data and issue a report on the findings in October of 2007. It will eventually be posted on the NYSILC website, www.nysilc.org, under the Housing Institute.
